Mentor support
At 3Belle.Ami Health
Each Young Person or Adult is matched and allocated a mentor to work with on a one to one basis to achieve set goals, and this key worker will be the first point of contact for the Individual. The levels of support required will be determined by:
- Initial assessment
- Local Authority recommendations
- Progress observed throughout the 'Leaving Care' transition
